Partnerships in Action The Agency for Workforce Innovation implements Florida’s VPK program along with our important partners in Florida’s Early Learning System, including: • Department of Education • Department of Children & Families • Early Learning Coalitions • Florida’s VPK Providers
Constantly Improving The Agency is committed to continually improving our business processes. As part of our improvements, we have recently added features to the three forms that providers use most frequently in the VPK program. • AWI-VPK 10 Statewide Provider Registration Application (February 14, 2007) • Draft Form AWI-VPK 11 Class Registration Application (May 24, 2007) • AWI-VPK 20 Statewide Provider Agreement (February 14, 2007)
VPK Forms Now Have Additional Functionality • Now you can save versions of the forms on your computer, including all the data that you input into the forms. • This means you can edit the forms any time in the future when information changes. • You can also electronically sign the forms and then email them as allowed by your coalition.
Additional Functionality • Now you can update version VPK 10, 11 or 20 at any time, creating updatable VPK 11 documents for each classroom, and it will be easy to make changes when you need to resubmit your information. • These enhanced forms are easier to fill in, easier to read, and easier to edit than they were in the past. • These enhanced forms can also be signed electronically and emailed.
About VPK Forms • The enhancements to the forms offer useful new features, but they require the same free software as you used with the old forms. • Most computers already have what you need -- Adobe Acrobat Reader.
Need Acrobat Reader? If your computer doesn’t already have the current version of Acrobat Reader that you need, you can download it free of charge from the Adobe website at: http://get.adobe.com/reader/

How to Use Forms Click on the link on the website to open the form you want. It should automatically launch in Adobe Acrobat. If it doesn’t open automatically, open your Adobe Acrobat software first and then click on the form.
How to Use Forms Save your document by using the File Menu, clicking on “Save As” and naming your document. Now the form is a PDF file on your computer, ready for you to input data.
How to Use Forms • Click in the highlighted fields. • Type in your information. • You can delete or add to the information you enter just like when using other types of documents. • You can “tab” to jump between fields, or you can move your cursor with your mouse. • Save your work and you will be able to print and edit it as needed.
How to Sign Forms • You can print out your form, sign and mail or fax it to your coalition: Early Learning Coalition of Sarasota County 1750 17th Street, Unit K-1 Sarasota, FL 34234 Fax: 941-954-4831 OR • You can electronically sign an enhanced VPK form by typing your name in the signature box. To transmit the form, attach your document to an email. Since email accounts are password protected, your email confirms that you are the sender of the document.
More Increased Efficiencies Ahead The Agency is working on many other ways to increase the efficiency of the VPK program forms. • Adding functions was only the first step in improving the VPK forms. • We are revising the current forms 10, 11 and 20 used in the VPK program to streamline the information requested and to make them easier to use. • The Agency has scheduled hearings about proposed changes to these three forms and rules 60 BB-8.300 and 301, F.A.C.
