  3. Situation 2 Student used a dictionary while taking the social studies test. Violation Page 7 – Classroom Materials Not Acceptable, paragraph 3
  4. Situation 3 Testing contractor discovered four Student Response Booklets with Post-it notes. Handwriting on Post-its didn’t match student writing. Teacher identified writing on Post-its as her own. Changes in answers appeared to be influenced by notes. Violation Page 7 – Administration Practices Not Acceptable, paragraph 1 & 2
  5. Situation 4 Scribe wrote answers on sheet of paper for a student with disabilities who can have a scribe. Student was required to copy answers into a Student Response Booklet as he had done throughout the school year. Not a Violation Page 18-19 – Inclusions Use of Scribes
  6. Situation 5 Teacher visually scanned student test booklets at the end of the day to make sure scrap paper was removed and to see if students had answered all questions. He noticed one student had written about plans to commit suicide. Teacher photocopied the student’s entry and took it to the principal. Violation Page 5 – Test Security Acceptable, paragraph 5
  7. Situation 6 Student began test then left for a doctor’s appointment. When student returned she was allowed to complete test. Not a Violation Page 10 – Administration Practices Not Acceptable, paragraph 1

  15. Situation 14 Student has a learning disability that significantly impacts her reading ability. Her IEP states materials may be presented orally.   17. Situation 16 The district has three new students that have been identified as needing specific accommodations to be included in a 504 Plan.
