  4. EXPECTATIONS & RESPONSIBLITIES SPORT ELIGIBILTY • Updated physical • Permission/Handbook Form - parent permission - review athletics handbook - 30 day medical review • Medical Authorization Form - self-carry medication

  7. EXPECTATIONS & RESPONSIBLITIES TRANSPORTATION POLICY • Bus provided weekdays • All team members required to ride bus to every contest • One email must be sent to the AD and coach if daughter is not returning on the bus for the season or for specific contests
