    2. INTRODUCATION Pharmacy dep. are responsible for the safe and effective use of drugs. The safe and effective use of drugs requires clinically competent pharmacists, system and services to deal with all patient care areas. Since the mid-1960’s SP have proven to be an important action plan to achieve the this goal SP puts pharmacists and drugs close to the end users and gives drugs and DI in an effective manner.

    3. PATIENT NEEDS Medication Errors: Reported 8-20% medication error rate. ME def. as the wrong patient, wrong drug , wrong dose, wrong route, and wrong time. Drug effects: Adverse drug reaction (5-20%) Prolonged hosp. Drug-modifying lab. Tests. Drugs inducing blood dyscrasia. Drugs affecting live function. Drugs interactions. Prescribing errors: Incomplete orders, inadequate lab. test monitoring, and misinterpretation of levels

    4. NURSING NEEDS Nurse plays a major role in the hospital medications system. Without adequate pharmacy support. Which drug-related activities should be preformed by the nurse? Can the nurse maintain sufficient knowledge about all medications? SP more readily provides the drug-related activities. Can improve the efficiency of the nurse.

    5. PHARMACIST NEEDS Time in a centralized pharmacy is likely to be dominated by dispensing duties. Professional relationships of the pharmacist with physicians, nurses, and patients limited. SP environment, pharmacist is able to relate directly to a patients drug therapy needs. Pharmacist can can develop expertise for particular patient care.
