Integration … by a Nurse Practitioner • Georgetown, DE : Rural State Health Department • Clinic flow … clients in/out within 1 hour (family planning visit, including Rapid HIV testing) • HIV is just part of the comprehensive visit and risk assessment • Approximately ½ of all clients test
Integration: By an Agency • Baltimore Maryland, Baltimore City Health Department: Health Teen and Young Adult Program • Commitment to staff training • HIV Testing Consent form sits at reception along with medical history and other required forms • Expectation that clients will test, conventional or Rapid • From 4/1/06 – 3/31/07 there were 2150 clients seen, and 1431 HIV tests performed (66%)
Integration: By a Grantee • Philadelphia, PA Family Planning Council, Inc. (FPC) • 27 provider agencies, with a total of 83 health centers • Agencies receive on-going data on their testing numbers, and will be audited at time of site visits.
FPC Developed a Policy which includes the following language: (pending Board approval) “Voluntary and confidential HIV Testing is an integral part of family planning services: therefore, all family planning sites must be equipped to provide routine on-site confidential testing, education/counseling, and referral … In addition, screening for HIV infection must be offered routinely to all patients minimally once a year.”
Some Tips: • “Triage” high-risk clients for more extensive counseling • Use open-ended questions to access the client’s knowledge • Perform all blood work simultaneously in order to prevent unnecessary needle sticks • If performing rapid testing, let test process while physical exam is being given • Cross train staff
More tips … • Eliminate staff biases: around age, race, gender and sexual orientation • Know your state laws, and be creative • Revise agency policies and procedures to reflect • Create a culture where there is an expectations for testing. Look at theories around organizational change
