Derrick & Well Ops Incidents • Pipe-racker roller pin retaining block (1kg) fell 15m to rig floor • Tong counterbalance line sheave (11kg) came free & fell 30m to rig floor (securing pin backed out) • Stand of drill pipe (840 kg) slipped out of racker arm & dropped 1m to rig floor (operator error) • Lump of lubricant (2kg) fell to rig floor from crown block (51m) • Tugger wire snagged on blocks, wire parted & fell to rig floor • Recovering wireline prong (1.7kg) from test tree, tool detached and fell 10m to rig floor • Picking up a joint of 3-1/2” drill-pipe, a drift (2.7kg) fell out 10m to the rig floor
Crane & Lifting Operations • Accommodation service lift cage dropped to lower level; hoist rope severed • Moving containers, safety latch (0.7kg) from crane main block detached and fell 26m to deck • Picking up waste skip, slings ejected contents which fell 10m over handrail to deck below • Moving diesel tote tank, it snagged and dislodged handrail which fell into sea • Moving container, it struck handrail, opening door; battery (25.7kg) inside fell 3.7m to work platform
Other Dropped Objects • Mobile toilet roof panel (35kg) detached by helicopter downdraft & fell 2.5m (corroded bolts) • Steel plate (5kg) from flare stack fell 60m to derrick monkey board, dislodged by high wind • Redundant light fitting (25kg) detached and fell 9ft to landing after painter brushed against it • Helicopter downdraft dislodged packing case from LCC roof 7.5m to walkway below • Shackle lying loose on drum fell over side of jack-up onto deck of platform below • Replacing grating (46 kg), crew lost grip and dropped it through space 10m to deck below
