Now that you are clear that I’m NUTS, some scenarios… • Like before only more so… (next verse worse than the first) • “The Great Pretender” (on political correctness) • Let’s get real!! (but it’s too hard!) • There’s got to be a pony in here somewhere! (the eternal optimist)
More of the same only worse • Reimbursement games • Dollar driven • Fighting over scraps (remember J. L. Seagull?) • Non-system (error-prone and all that) • Uninsured? Just a fact of life (in the market) • Escalating costs, tanking value • Competition is king and win-lose is life
Let’s pretend... • Best health care system in the world! • Liability reform! • Fraud!! • Budget neutrality! • It’s just about the money.. • Sound bites for politics • Just a few small fixes.. • Pay for performance will fix everything!
Let’s get real (about the current state) • Perverse reimbursement • Exclusion • Financial rationing • Administrative waste • Inappropriate care • Physician autonomy: top operational value? • Duplication • Non-”STEEPE” • Technology-challenged • And much, much more
