Pay to Participate The 2014 – 2015 Registration Fee Structure: • High School Sports: • $100.00 One-time fee for the 2014-15 school year. • $50.00 One-time fee for the 2014-15 school year IF the student qualifies for reduced lunch. • FREE for the 2014-15 school year IF the student qualifies for free lunch. • $300.00 Family Cap • Middle School Sports: • $50.00 One-time fee for the 2014-15 school year. • $25.00 One-time fee for the 2014-15 school year IF the student qualifies for reduced lunch. • FREE for the 2014-15 school year IF the student qualifies for free lunch. • $150.00 Family Cap Fee Date Deadline: January 5, 2015. If a payment plan is needed contact the Athletic Office to set up a plan. The participation fee is non-refundable & does NOT guarantee playing time.
Forms The following forms need to be turned in ASAP. • Physical Form – not allowed to participate until it is turned in to Athletic Office. • Medical Release Form – can’t participate in a contest until it is completed. • Athlete Code of Conduct – can’t participate in a contest until it is completed. • Concussion Acknowledgement Form – can’t participate in a contest until it is completed. • Participation Fee – Due Date is January 5th.
Concussion Law When in doubt sit them out! The concussion law became effective June 30, 2013, and requires all levels of schools and youth sports organizations to educate, train and collect forms for non-MHSAA activities including physical education classes, intra-mural and out-of-season camps or clinics. • If a student is withheld from competition due to a suspected concussion, he or she may not return at all on that day and only on a subsequent day with the written clearance of aMD or DO.
Transportation On regular school days, all team members must travel and return from away contests with the team, except with prior approval of the coach and athletic office. A travel release form must be completed prior to the date of the contest. If a request to ride home with the parent is granted, the coach must meet with the parent before allowing the parent’s child to leave. Athletes may not ride with anyone other than their parent or guardian.
