CAM • Complementary and Alternative Medicine use increased from 25% to 1990 to 42% in 1997 • Total visits to CAM providers in 1997 (629 million) exceeded total visits to all primary care physicians (386 million).
Arthritis • In the United States, about one out of every six people (nearly 43 million) is affected by arthritis
Medical Therapy • COX- 2 selective nonsteroidal anti-inflammatory drugs (NSAIDs) such as celecoxib and rofecoxib are now to be considered first-line drugs of choice for treating moderate to severe pain, and mild pain associated with inflammation.
COX-2 • The cardiovascular effects of COX-2 inhibitors were evaluated in a review of multiple studies published in the August 22 edition of the JAMA This review focused on two major studies in particular, each with over 8,000 participants: the Vioxx Gastrointestinal Outcomes Research Study (VIGOR) and the Celecoxib Long-term Arthritis Safety Study (CLASS).
COX-2 • VIGOR showed that Vioxx increased the risk of cardiovascular events, such as heart attack or stroke, almost two-and-a-half times. Two other smaller studies also suggested a relationship between Vioxx and these conditions. CLASS indicated an increased risk for cardiovascular events in COX-2 users, but was inconclusive because of minimal data.
