The focus of this particular slideshow is Section 2 – “Organization” City of Fort SaskatchewanSafety Qualification Form • Thank you for choosing to work with the City of Fort Saskatchewan! • This slide show is intended to help walk you through filling in the form and providing examples of what attachments will be required.

  3. Section 2: Organization What service does the vendor provide?

  4. Section 2: Organization Provide the WCB Industry Code…

  5. Section 2: Organization …followed by the description (Industry Title). If you have more than one Industry Code, keep adding until complete. If you fill will the sheet, please add remaining as an attachment. See Attached

  6. Section 2: Organization Are you self employed or do you have workers? If you do employ workers at some point or always, click here. If you employ no other workers at any point throughout the year, click here.

  7. Section 2: Organization Are sub-contractors used by your company? NOTE: If you are awarded a project and intend to use SC, the City must be notified in advance of any work beginning City of Fort Saskatchewan Definition: “Sub-Contractor – shall mean any employer(s) hired by a consultant, contractor or supplier or their sub-contractors to assist the consultant, contractor or supplier in the conduct of the project on behalf of the City.” If you do use SC and would use SC for City projects, click here. If you do not use Sub-Contractors (SC), click here.

  8. Section 2: Organization If you indicated that you use sub-contractors, do they have their own safety management system (SMS)? If they do not, they must be aware that they will have to abide by your SMS or that of the City. If you didn’t use SC in the first place, just leave these as unchecked please. If they do, that’s good to know.  We will require more information about any SC after a contract is awarded. Safety Management System of the Sub-Contractor

  9. If so, which standard? 9001:2015 – Quality Management 14001:2015 – Environmental Management 45001: – OH&S Management Etc… Does your company hold certification for ISO? (If you want to know more about ISO certification, click here) Section 2: Organization If you do not hold ISO certification, no worries. This may become a future consideration for Prime Contractor candidates. Other Certificates of Corporate Achievement? 2015 14001:2015
