PUBLIC HEALTH ROLE IN TRANSITION • RTOI Spina Bifida Projects • Longitudinal study of young adults with spina bifida at Partners for Inclusive Communities, University of Arkansas, looking at obesity, social isolation and depression as outcomes • Cross-sectional, multi-site study of young adults with spina bifida, coordinated at University of Indiana, looking at quality of life and independence as outcomes
PUBLIC HEALTH ROLE IN TRANSITION • Transition summit held in September 2007 • Developed list of issues for youth with spina bifida • Those attending the meeting will form working groups to develop list of ‘best’ and ‘promising’ practices, as well as most pressing research questions
PUBLIC HEALTH ROLE IN TRANSITION • Manual of current ‘best practices’ in transition at selected clinics under development • Exploring a Transition Office at CDC to oversee transition efforts in spina bifida and other chronic conditions
HEALTH DISPARITIES • Chartbook highlights the following: • Obesity, smoking and physical inactivity • Implications for chronic disease programs in public health • Implications for primary health care and capacity building
