Create and Edit Form 4 for Appraisers (Primary and Secondary Care)
Appraisal Form 4 (Summary) • Form 4 is a summary document of the appraisal discussions • It is used by Appraisees as proof of their appraisal • For Quality Assurance purposes, only the Appraiser can draft the Form 4 • Appraisee reviews and approves forwarded draft
Overview: Form 4 process Appraisee Appraiser Process completed: automated email confirmations sent Complete Appraisal Docs (Forms 1 - 3) Reviews submitted Appraisal Docs Agrees and Signs Agree Confidentiality Statement (MUST) APPRAISAL INTERVIEW Disagrees Signs and Forwards Submit Appraisal Forms (MUST) Drafts Form 4 summary Requests changes
Navigate to interview details • Navigate to Interview details in order to create Form 4 (for that interview) • Use Powerpoint slideshow “Look up Appraisal details” as reference • Quick recap: • Login to SOAR (as Appraiser) • Click on “Interviews” • Locate interview and click on corresponding notepad icon
When in Interview details, click on “Create Form 4” button • This creates the Form 4 and you will automatically be taken there
Three sections to complete: • Summary of Discussion • Summary Assessment of Supporting Information • PDP • Click “expand” to open each section • You are not expected to complete this in one sitting, use the Save button as required • When finished, click “Approve and Forward to Appraisee” for their review and sign off • Form 4 will then be locked from further editing • When Appraisee signs off / approves Form 4 – interview is completed • If Appraisee makes Change Request, Form 4 will be unlocked for you to make changes as appropriate
In section A, you are asked to provide a 0, 1 scoring against each of the Core Elements • The scores will populate the table in section B • Until Form 4 is signed off, the scores on section B will remain as “TBC” • GP Form 4 has an additional question, asking if the appraisee is on-track to provide sufficient info for Revalidation • If the Appraisee has completed the Appraisal Forms correctly, PDP (section C) should be auto-populated – leaving you to amend as required following appraisal discussions • If not, you can add the learning items in PDP section manually instead • GP PDP more structured/reflective • Secondary Care PDP more straightforward
