ACORD 125 COMMERCIAL INSURANCE APPLICATION PAGE 1 Date – Date of submission Agency – You must have The Ahbe Group under agency. You can add your agency name as shown. Carrier – Unless it is a renewal, Carrier will be blank. Indicate coverages requested. Status of Transaction – Choose quote. Package Policy Information – Proposed effective date/expiration date. Make sure there is adequate time to quote. Applicant Information – Please put the legal name of the company. You can include a dba. Mailing Address – Address is required FEIN or Social Security Number - Required for submission. Include phone #, email address and website. Date business started is required. Premises Information – Include address, owner or tenant, yr built, employees, revenues and % occupied. Nature of Business/Description of Operation – please be specific. Company websites are a good source. Page 2 Must answer all questions #1-12. Signatures – signatures will be required for binding. Page 3 Prior Carrier information – Type of policy is it, carrier it written through, current coveragesand previous/ target premium. Loss History – loss runs required for most binds.
ACORD 126 COMMERCIAL GENERAL LIABILITY SECTION Page 1 Date – Date of submission. Agency - You must have The Ahbe Group under agency. You can add your agency as shown. Applicant - Please put the legal name of the company. You can include a dba. Effective Dates - – Proposed effective date/expiration date. Make sure there is adequate time to quote risk. Coverages – Claims made or Occurrence policy. Limits – Proposed insurance limits. Schedule of Hazards – By location, classification of risk. Include Payroll (separate owners payroll) and annual revenues. Page 2 Answer questions 1-6 Describe the Type of Work subcontracted - Dollar and percentage amount subcontracted, employees. Products/Completed – Products and annual gross sales. Answer questions 1-10. Page 3 Additional Interest – include additional insured, loss payees, etc. General Information – Answer questions 1-15. Page 4 General Information (cont) – Answer questions 17-20.
ACORD 127 BUSINESS AUTO SECTION Page 1 Date – Date of submission. Agency - You must have The Ahbe Group under agency. You can add your agency as shown. Applicant - Please put the legal name of the company. You can include a dba. Effective Dates - – Proposed effective date/expiration date. Make sure there is adequate time to quote risk. Driver Information – List all drivers, DOB and Drivers license numbers. General information – Answer questions 1-15. Additional Interests – Add additional insured, loss payees, etc. Page 2 Vehicle Description – List all vehicles, year, make, model, vin #, etc. The acord 127 Business Auto Section is one area where underwriters will accept other forms or copies of existing policies or dec pages, especially where there are multiple drivers and vehicles.

Applicant - Please put the legal name of the company. You can include a dba. Effective Dates - – Proposed effective date/expiration date. Make sure there is adequate time to quote risk. Premises Information – Number of premises and buildings. Subject of Insurance – Property covered and coverage amount (building, personal property, etc.). Additional Coverages – Tools, equipment, etc. Property Description – Construction type, fire department info, yr built, area, improvements, alarms, sprinklers. Additional Interests – Include any loss payees or mortgagees. Page 2 Same for any Additional Buildings.
ACORD 130 WORKERS COMPENSATION Page 1 Date – Date of submission. Agency - You must have The Ahbe Group under agency. You can add your agency as shown. Applicant - Please put the legal name of the company. You can include a dba. Years in Business – Amount of years company has been in business. Type of company – Individual, corporation, etc. Locations – List locations and addresses. Effective Dates - – Proposed effective date/expiration date. Make sure there is adequate time to quote risk. Employers Liability – Include limits. Rating information – List classifications by employees and payroll. Page 2 Individuals included/Excluded – Include any owners to excluded/included. Name, DOB ownership %, wages and last 4 digits of owners’ social security number. Prior Carrier Information – Include prior carrier, premium and losses. Nature of Business - Provide detailed description of business operations. General Information – Answer questions 1-24.
ACORD 131 UMBRELLA/EXCESS SECTION Page 1 Date – Date of submission Agency - You must have The Ahbe Group under agency. You can add your agency as shown. Applicant - Please put the legal name of the company. You can include a dba. Effective Dates - – Proposed effective date/expiration date. Make sure there is adequate time to quote risk. Policy information – Occurrence or claims made, limits, retained limit (deductible). Primary Location – Address of location(s), annual payroll, annual gross sales, number of employees. Underlying Insurance – List the underlying coverage (Auto, GL, WC). Page 2 Care, Custody, Control – Description of real and/or personal property (value, square footage. Vehicles –List number of vehicles and use. Page 3 Additional Exposures – Answer questions 1-19. Page 4 Additional exposures (cont) – Answer questions 20-27.

Unscheduled equipment – Description and value of unscheduled equipment. Page 2 Scheduled Equipment – List any scheduled equipment (type, make, model, year, value etc.).
