  4. Why do we report? • To prevent recurrence by: • Collecting data • Ensuring Heads of Departments are aware and can investigate properly • Panning follow-up activities which tackle the cause/ of the incident

  5. How do we report? • Give as much detail as you can • Be factual – Do not speculate • Be clear – Do not be vague
