Addresses: Strategic Health Priority # 2: Access to Care and Health Disparities Essential Public Health Function: Links individuals who have a need for community and personal health services to appropriate community and private providers Subcontracts with community agencies to provide needed services to HIV + individuals Assures that priorities are determined by the community Provides technical assistance to subcontractors. Manages ARIES database Performs programmatic and financial audits Writes and manages grants HIV Administrative Services

  3. Aids Coalition of Coastal Texas (ACCT) Provides: • Ambulatory/Outpatient Medical Care • Case Management • Emergency Financial Assistance • Food Bank/Home-delivered Meals • Transportation Services • Drug Reimbursement • Health Insurance

  4. Bering Omega Community Services, Inc. • Oral Health Gulf Coast Center Connect Transportation • Transportation Services Gulf Coast Center – MHMR • Mental Health Services • Substance Abuse Services - Outpatient

  5. Also serves clients in Brazoria and Matagorda Counties.
