  1. Esettanulmány Focalis segmentalis glomerulosclerosis familiaris előfordulása Dr. Degrell Péter egyetemi adjunktus PTE ÁOK II. sz. Belgyógyászati Klinika és Nephrologiai Centrum, Pécs

  2. L.A. 26 éves nő • 2005. augusztus • Hajhullás • Mko. csípő, térd, kéz kisizületi fájdalom • proteinuria (1g/d) • visszatérő makroszkópos haematuria • hypertonia • ANA: 30,3 IU/l (<30) • C3: 0,86 g/l (0,9-1,8) • Édesapja vesepótló kezelésre szorult → vesebiopsia (2006. jan.)

  3. S.B. 19 éves nő • 1992-től visszatérő makroszkópos haematuria • Gyermekgyógyászaton gondozták – 2000-ig • 2006-ban fejfájás, hypertonia miatt kezdték vizsgálni: • Proteinuria: ~ 5 g/l • Se-albumin: 31 g/l • Triglicerid: 2,0 mmol/l, koleszterin: 6,5 mmol/l → vesebiopsia (2006)

  4. Cs.E. 49 éves nő • 1977. terhesség alatt proteinuria és haematuria, ami szülés után is megmaradt • 1978. vesebiopsia más intézetben: terhességi nephropathia (EM nem történt) • 1984. toxaemiás terhesség (proteinuria, ödéma, hypertonia) • 1985. uterus exstirpatio • 1999. óta nephrologiai gondozás • növekvő proteinuria miatt → ismét vesebiopsia (2006) • Egy nővére szintén proteinuriás, más intézetben vesebiopsián átesett, eredmény nem ismert.

  5. Vese biopsziaImmunfluorescencia IgA, IgG, IgM, C3, C4, C1q, fibrinogén,albumin, kappa, lambda,(DAKO): negatív

  6. Focalis segmentalis glomerulosclerosis=FSGS

  7. 1978 vesebiopszia: nephropathia gravidárum 2006 vesebiopszia: Focalis segmentalis glomerulosclerosis=FSGS ??

  8. Testvérének ismert vesebetegsége van, vesebiopszia történt évekkel ezelőtt más Intézetben. Dg: ? Diff. dg: Familiáris FSGS?! De még ezelőtt…

  9. EM diagnosis: ALPORT szindroma
