Oct 18-04 Safety Meeting REG & GLAST LAT 4.1.7 DAQ & Electronics Gunther Haller haller@slac.stanford.edu (650) 926-4257
Status • Accident has occurred • Machine is down • Work is suspended • Type A investigation has started • Estimated duration: 3 weeks • Interviews, assessment of safety • knowledge, implementation, culture, attitude • Safety must come first • Must change our culture • Safety must come first • Generate ideas and steps to take charge of the safety culture in our group and in our programs • Suspension removal incremental • Depends on progress made towards changing culture • Gunther will meet daily with research division head • Result is daily update what is permitted and what not
Safety • Safety is first • Assess what we do is safe, what is not safe • Is the environment safe • Are we working safely • Need to get “buy-in” of members that safety is most important for all of us, at SLAC *and* away from SLAC
Directors Message • Message from Jonathan October 18, 2004 • As you all know, we had a very serious accident at SLAC last week involving an electrician. We re-extend our heartfelt concern to his family and continue to pray for his rapid and full recovery. • The rationale for the current work suspension is neither to assert, nor to imply, blame to any one of you for this accident. That is not my aim, nor would it be appropriate. Rather it is to remind us all that there are serious hazards in our workplace and to provide an opportunity for us all to actively engage in a process that re-invigorates our strong commitment to safety so that never again will we have to experience at SLAC an accident of this magnitude. • I thank you all for your full and unswerving support. • Jonathan • Directors Memo • http://www-glast.slac.stanford.edu/Elec_DAQ/misc/SafetyFirst_DirGuidanceMemo
Morning • AM: • Everybody read relevant documents as listed by directors memo • Completed JHAM’s and AHA’s if not already done so • See Traci Kawakami if questions • Discussed filled-out • JHAM’s • AHA’s • MSDS’s, Material storage if applicable • Discussed individual training including CPR • 219 General Orientation to ES&H • 115 GERT if applicable • 136 Lock & Tag – Affected people; 157 Lock & Tag; if applicable • 239 Electrical Safety – Non-Electrical Worker • This week for everybody • 251 Electrical Safety- R&D Equipment • Everybody working in lab-space • 274 Electrical safety – Low/High Voltage (if applicable) • 138 CPR or 138R CPR recert. • Evaluated environment, • improve house-keeping to make work-place as safe as possible • Collected action items

Specifics • ES&H good practices • Already shown relevant slides of most relevant ES&H Safety Manual last week: • http://www-group.slac.stanford.edu/esh/manuals/eshmanual.html • Medical • Hazards • Evacuation • Electrical • Since most people work around electrical/electronics equipment and won’t be able to take class in the next couple of weeks. • Went over relevant slides from Electrical Safety for R&D equipment • When suspension of work is over • Clarify that exposure to above 50V need Gunther’s assessment that it is < 5mA/10J before proceeding. • If above 5mA/10J, need Hot Work permit. • Identify and mitigate risk of exceptions (if any). (e.g. insulate potentially exposed contacts, etc).
Examples • GLAST/EXO/BaBar/general lab electronics • Will distribute details of potential exposure • E.g GLAST: Tower Power Supply/TEM has > 50V but < 10J and < 5 mA • More to follow
Discussion • Safety must come first • Must change our culture • Safety must come first • Generate ideas and steps to take charge of the safety culture in our group and in our programs
