Let’s Increase Our Census Adults with Developmental Disabilities: How it can work in an Adult Day Center Setting.
Origination of Idea • The center was currently serving clients that had a primary diagnosis of Developmental Disability • Current programming was integrated with seniors with limited functional activities directly related to diagnosis • Due to past experience in residential and DT services for DHS and IDPH, I felt we could expand our program
Origination continued… • That began my investigation into the how the pay rates differentiated from IDOA, DORS, and the various other payor sources that the agency currently accepted. • This presentation will give you the framework to increase your centers census. S T A R T
Funding/Payor Sources • Waiver Services 35U is the funding sources that is often not being utilized in Adult Day Centers • In order for a center to bill under 35U the Adult Day Center must meet the following requirements: 1. Current contract with Department on Aging 2. Must be in compliance with Rule 240 3. Community Care Program (89 III. Admin. Code 240)
Where do you Start? • DHS Office • Residential Agencies that provide CILA and/or Home Based Services • Office of State Guardian There are several key professionals that a Center’s Director/Marketing staff should focus on to start developing these relationships.
DHS Office • Follow the below link to identify the office that serves your area: http://www.dhs.state.il.us/page.aspx?module= • Important to connect with an ISSA (Individual Support Service Advocate). • The ISSA agent serves a similar role in DHS as the Care Coordinator does with the CCU.
Residential Agency • Identify local Residential Agencies in your area by going to the following website: www.dhs.state.il.us/page.aspx?item=56783 • Once an agency has been identified, request to speak with the QIDP (Qualified Intellectual Disability Professional) • QIDP is the case manager for the individual who is receiving services at the specific agency !
Office of State Guardian • Many clients served in a residential agency have state appointed guardians. Key Topics to Discuss with Professionals Provide specific examples of programs that your center provides for different cognitive levels. Provide the professional with a “daily set schedule” that provides structure to the clients that attend. Provide examples of the type of documentation that is used at your center for each client.
Key Topics to Discuss with Professionals continued… Provide basic outline of training that is provided at the Adult Day Center. Transportation process for clients who attend the center. Nursing services and the hours that your nurse is available during the day.
Programs • Examples of programs that would meet the requirements of a DHS funded program. 1. Money Skills 2. Sorting 3. Exercise 4. Learning 5. Wii 6. Social Skills
Training • Provide detailed explanation on the type of training as well as on-going training that is provided to all staff. • Express willingness to train staff on specific needs of each and every client that attends the Adult Day Center
Transportation • Provide them examples of what type of transportation you utilize at your center. • Wheelchair accessible buses/vans
Nursing Important • Nursing is the area that will be one of the best selling points for your center. • Majority of other DT sites do not have the options of either an RN or an LPN available during the day to provide medication administration, insulin, injections, etc.
Reimbursement Rates • Reimbursement Rate for clients who attend your program through the 35U is: $12.34/hour includes transportation • Maximum hours per year is 1100
Summary/Results • The Adult Day Center increased clients served from this specific age/cognitive functioning by 50% in 8 months. • Since MCOs began referring clients to the center, 4 out of 8 referrals have been clients with a primary diagnosis of a Developmental • Disability.
