Take the paper order sheet as an example.” • “The clinician writes a set of orders for care, diagnosis and treatment when a patient is admitted to the hospital. Thereafter, orders are added, stopped or changed one at a time. After the patient has been in the hospital for a short time, there is no one place where anyone can see all of the active orders for the patient. As a result, it has been common to require by policy that all orders be re-written on major transitions such as transfer of a patient from intensive care to intermediate care.”
