YOUR NDIS ROLLOUT The Western Sydney district started full NDIS transition from 1st July 2016
ELIGIBILITY • A permanent and significant disability; a needs based assessment • You must be an Australian citizen or permanent resident. • Enter the scheme before 65 years • Existing service userswill have a smooth transition to the NDIS; the NDIA will contact you your services will help you with information you can contact the NDIA • http://www.ndis.gov.au/ndis-access-checklist

PLAN MANAGEMENT • Agency Managed - Support providers deal directly with the NDIA • Registered Plan Manager - Funding in your plan is allocated for a 3rd party to manage the finances in your NDIS plan • Self Managed - You (or your nominee) manage the funds and transactions for services and supports • Automated Payments - (for transport only) Funds can be deposited weekly, fortnightly or monthly into a nominated account • Combination – Your choice of a combination of these options can be used to meet your individual needs
