DISCLOSURES of CONFIDENTIAL DATA REQUEST FOR PHI ACCESS After releasing beneficiary information to any person or entity external to DHCS, DHCS programs should document the release including the date of release, beneficiary’s name, requestor’s name, address, and phone number, and the MEDS ID, CIN, or SSN type of information that was released. Releases to other divisions with DHCS do not require documentation (though it is always a good business practice). Sample Access Request Log is shown below: 1
