Helpful Hints • The application process requires detailed information • There are box by box instructions for the • MAD 312 for providing services, paid by MCO, claims submitted to MCO • MAD 355 for providing services, billing directly to Xerox or under MCO • Before proceeding to the portal screens, you may want to: • Review the box by box instructions for the application, or • Print the box by box instructions

Providers with MCOs • If you are providing services under an MCO, billing the MCO or receiving a capitation and will be paid by the MCO, use the MAD 312application • If in private practice and providing services under the MCO, you may already have your provider identification. • If in private practice and providing services under the MCO, possibly billing Xerox for some clients, use the MAD 335 application
MCO providers note: • Provide services and paid by MCO, use Mad 312 • If private practice billing directly and also with MCO, use MAD 335 Check one of the types and one of the 3 options below
Before you move on • You may want to review the box by box instructions for the: • MAD 335 groups, MCOs, individuals who would receive payment, if provider bills Medicaid (Xerox) directly. • MAD 312 Individual applicant within a group, or if paid by an MCO, such as Behavioral Health providers
You will receive a unique reference number on the next screen, whether you continue or exit You may want to gather documents to upload attachments on subsequent screens
One final screen to submit the application on line, then a summary page with your tracking number
