Creative Customizing of CDEMS Methods For Distinguishing Single Or Multiple Chronic Conditions Jesi Ogo The Polyclinic
Project Goal • Enhance the CDEMS forms capabilities by creating a Diabetes or Cardiovascular or both DM/CV specific information box linked/tied to a patients diagnosis
Description Sure Jesi, no problem, while we’re at it, why don’t we just conquer the world and end world hunger! • Phone call to Dusty BLAH BLAH BLAH AND BLAH BLAH BLAH AND How could we do that?
Description cont. • Using Word and Paint to do the heavy lifting.
Description cont • How did that get there?
Description cont. • . . . And what language is this??!!
Case 1 Label33 – (graphs page) This is a Dusty label, incorporated this label into instructions as this only shows up if DM-2/DM-1is checked. There is no “Last Done” column for CVD Image137 lblDM Outcomes
Case 10, 20, 30 ListCV – (graphs page) This is a static CV specific “Recommendations For The Patient” label row sourced to jtblCV Image139 lblCV Outcomes cont.
If DM – 2 or DM-1 and any one, two or all three CVD conditions are checked . . . Case 11, 21, 31 ListCVDM – (graphs page) This is a static CV/DM specific “Recommendations For The Patient” label row sourced to jtblCVDM Image140 lblDM lblCV Outcomes cont
Things to Remember • Always make changes in a static copy of CDEMS • Text boxes have to be labeled correctly in the code and placed in the correct location • Don’t be afraid to ask THAT question and if confident enough make that move
THANK YOU!
