Components of Person Centered Planning for High School & Transition Students • Gain a basic understanding of transition planning and future directions • A pre-planning meeting will help the student, family, and school decide the best circumstance and logistics for the planning meeting • A team is developed around the student to assist in planning and implementation • A Person Centered Planning meeting is facilitated by an outside facilitator such as a staff person from WiSe, or it may be an educator or employment provider • Person Centered Employment Goals are aligned with IEP goals • The school and adult service system work with the student and family to reach identified goals WiSe: www.theinitiative.ws
