  12. How? • After research in which different instruments were tested, the Belgian government chose the interRAI instruments

  13. The interRAI instruments in Belgium • Two objectives • An instrument thathelps to develop a personalized care plan thatenhances the quality of care for and the quality of life of the person • An instrument thatcanbeused in differentcontexts (home, nursing home, hospital)

  14. InterRAI-Suite • interRAI Home Care • interRAI Long Term Care Facilities • interRAI Acute Care • interRAI Palliative Care • [interRAI Mental Health] • [interRAI Community Mental Health] • ….

  15. Languages Dutch French German Lot of effort to make sure all is identical, but absolutely worth it

  16. BelRAI - Web-based software Single virtual host information system All caregivers involved have access, with a lot of security measures (privacy) Training With wiki-site: to make sure they continue to assess correctly / keep info up to date In 3 languages

  17. Is everybody in Belgium using this? IS EVERYBODY IN BELGIUM USING THIS ? • Not yet, but we’re well on the way • In 2010-2011 pilot in 4 geographical areas with different nursing homes, hospitals and home care services • Start with most motivated • Government decision to structurally finance BelRAI in June 2012 • Implementation plan pending

  18. Also All over the country 66 projects ‘innovation in care’, aimed at preventing or postponing residential care are using the belRAI HC (Protocol 3) Research project with belRAI PC And some organizations who use the instruments out of free choice

  19. Advantages? • Continuity of care • Holistic, full profile of client • Everybody works together and communicates • Prevention: • Of problems becoming worse • Of (re-)hospitalization • Of having to move to residential care Overall: better quality of care

  20. And: good data! Better for targeting the right populations Better for everyone e.g. recent Health Council of Canada Report

  21. Hurdles to be taken Sense making to the users so that they see it as differing from practice as usual (We already do it ... ) develop a shared view of its purpose understand how it will affect them personally grasp potential benefits

  22. Hurdles to be taken Take time forpreparatoryactivities, policychange, dissimination of information Develop participation of the users :Engage them into the process of development

  23. Obstacles in organisation of care Budget Personnel Legislation and regulatory frame Demand of the client and family ....

Stimulating policy Accepted by the sector with growing awareness of : More time for the core business Scientific basis More diversity in the approach Be patient and Listen Change in culture → not easy, takes time Budget Many people/organizations/governments involved Need to work on integration of other instruments and software
