  1. NTC ROTATION 13-01 SAFETY ALERT SAFETY ALERT SAFETY ALERT !!! FLARE MISHAP!!! ENSURE WIDEST DISSEMINATION AND POST ON BULLETIN BOARDS While conducting a night live fire exercise, a crew member prepared a M127A1 white star, parachute flare and placed it on the floor of the MATV. The flare initiated inside the vehicle resulting in personal injury and minor vehicle damage. Injuries included second-degree partial-thickness burns to back and inside of inner thigh. • FOLLOW PROCEEDURES – READ AND UNDERSTAND WARNING AND CAUTION STATEMENTS – • NEVER PRE-ARM PYROTECHNICS. • ARM ONLY WHEN YOU INTEND TO USE. • ALWAYS HOLD TUBE WITH EJECTION END UP AND POINT AWAY FROM ALL VEHICLES AND PERSONNEL. !!! FLARE MISHAP!!!
