  2. Med 3 • If period of incapacity > 7 DAYS • Closed certificate if illness< 14/7 • Open certificate • CANNOT BE BACKDATED

  3. Med 5 • BACKDATED certificate based on previous consultation with patient • Other recent doctors report • Can be backdated for ONE MONTH and forward dated for ONE MONTH

  4. Med 4 • After 28 weeks incapacity • Requested by DWP, long form requires gp to give details of medical illness/condition

  5. Med 6 • VAGUE DIAGNOSIS if deemed harmful to patient or detrimental for employer to know

  6. RM 7 • If you doubt disability of patient, form sent to Jobcentre Plus to review patient sooner

  7. DS1500 • Terminal illness, not expected to live >6/12

  8. SC 1 • Self certificate, for SELF EMPLOYED so not eligible for statutory sick pay, to claim incapacity benefit

  9. SC 2 • As SC1 but to claim statutory sick pay
