OCCUPATIONAL INJURY/ILLNESS REPORTING • All occupational injuries/illnesses must be documented on a “First Report of Occupational Injury/Illness” form. • The form is on the OEHS website http://www.som.tulane.edu/oehs/. • Employees are responsible for reporting occupational injuries/illnesses to their supervisors. OCCUPATIONAL INJURY/ILLNESS REPORTING • Complete both sides of the form within 24 hours of notification of injury. • Supervisor/Unit Head/PI reviews and signs form. • Supervisors are responsible for submitting the form to OEHS (Worker’s Comp) via campus mail (TW-16) or fax (504) 988-2196. • Follow instructions at the bottom of the form for proper distribution. OCCUPATIONAL INJURY/ILLNESS REPORTING • If possible, a copy of the completed form should accompany the employee to a medical treatment facility. • When immediate treatment is necessary, seek treatment. OCCUPATIONAL INJURY/ILLNESS REPORTING • If death, serious injury, or multiple hospitalizations occur, OEHS must be notified IMMEDIATELY at 988-5486. • During non-business hours, notify Public Safety/Security (TUHSC: 988-5555; Uptown campus: 865-5200) • See the OEHS Policies & Procedures Manual, Section 4 for more information. OUTSIDE AGENCY INSPECTIONS • Agencies dealing w/ Environmental Health & Safety may visit/contact the University regarding inspections, such as: • Governmental/regulatory agencies • OSHA, DEQ, DHH, State Fire Marshal, Fire Dept. • Insurance companies • Accrediting groups (JCAHO, CAP, etc.) • Inspections are usually unannounced. OUTSIDE AGENCY INSPECTIONS • Notify OEHS & Risk Management if an inspector visits or contacts you. • An OEHS representative must accompany all inspectors. • See OEHS Policies & Procedures Manual, Section 13 for more information. Tulane University - Office of Environmental Health & Safety (OEHS)
TULANE SMOKING POLICY • Smoking is prohibited in all Tulane buildings except in designated areas. • Smoking is not permitted in private offices unless the room is equipped with an individual HVAC system with a dedicated exhaust system. • See OEHS Policies & Procedures Manual, Section 26 for more information. SPECIAL PURCHASES • OEHS approval is required for purchases of certain items. SOME examples are: • Hazardous materials/equipment (or services) • Radioactive materials/radiation producing equipment • Lab hoods (BSCs, LFWS, chemical hoods) • Personal protective equipment (respirators, etc.) • Equipment used to store/dispense flammable liquids • See OEHS Policies & Procedures Manual, Section 31 for more information. EMERGENCY PLANNING & RESPONSE • Each Unit should prepare (& periodically review/revise) an Emergency Plan specific to their location & needs. • Plan should include: • Inventory of equipment, chemicals, supplies • Updated staff contact information • List of critical documents & equipment • Arrangements for storage or on-site protection • Site-specific SOPs Tulane University - Office of Environmental Health & Safety (OEHS)
EMERGENCY ACTION PLAN (EAP) • EAP’s are being developed by OEHS, Emergency Preparedness, Security, & Facilities Services for each building • EAP includes: • Procedures for reporting emergencies • Description of fire alarm system (activation, signal, elevator recall, etc.) • Building evacuation & re-assembly area • Assistance of persons w/ special needs • See OEHS Policies & Procedures Manual, Section 1 for information • Review the EAP for your building!! BASIC SAFETY • Wear Tulane ID at all times. • Obey warning signage. • Use a ladder or step stool to reach high objects. • Keep walking surfaces clean and in good repair. • Report safety hazards to supervisory personnel. • See OEHS Policies & Procedures Manual, Section 10 for more information. CONTACT OEHS… • To report: • Serious or imminent hazards or unsafe work practices • Emergencies (major spills of hazardous materials, fire, serious injury or accident)* • Ergonomic problems • Asbestos concerns • To coordinate the purchase of equipment requiring OEHS approval • If an outside agency visits or contacts you *During non-business hours, notify Public Safety/Security Tulane University - Office of Environmental Health & Safety (OEHS)
