Care Coordination Role Daily collaborative interactions with physicians, nurses and ancillary staff to coordinate/facilitate the delivery of high quality, individualized, cost-effective patient care. • Attend Daily Rounds • Assist with expediting priority services to facilitate medical clinical decision-making • Functions as a resource: • Clinical documentation to support medical necessity of services being provided • Expected LOS • Identification of potential discharge planning needs • Quality care/risk management issues • Interpretation of federal regulations as they apply to clinical situations. • Assist & support the discharge planning process.
Care Coordination Role • Analyzes patterns and trends in patient care for quality improvement opportunities • Identify & monitor resource utilization, process & system delay • Report results to appropriate designated departments/committees with recommendation for performance improvements
