Fatality #1 - January 9, 2010Falling/Sliding Material - Arizona - Copper Ore NECFreeport McMoRan Miami Inc - Freeport McMoRan Miami Inc • On January 9, 2010, a 20 year-old contractor laborer with 21 weeks of experience was fatally injured at a copper operation. Two track excavators were being used to place a 400-foot long section of 24-inch diameter plastic pipe into a pond. To facilitate the installation process, the pipe was placed on top of an adjacent section of pipe that was previously placed on the plastic lining of the pond. During installation, the pipe being installed misaligned and the victim and two coworkers attempted to remove the end of this pipe from the top of the existing pipe. At that time, the end of the pipe shifted, hit a coworker, and then struck the victim. The victim fell to the ground and the pipe landed on him. The coworker was not injured
Fatality #2 - January 26, 2010Powered Haulage - California - CementLehigh Southwest Cement - Tehachapi Plant • On January 26, 2010, a 59 year-old purchasing manager with 5 years of experience was fatally injured at a cement operation. The victim was struck by an over-the-road tandem trailer truck. The truck had been waiting to unload. When the truck pulled forward, another truck driver observed the victim under the second trailer of the truck and immediately stopped the driver. The victim was holding a cell phone at the time of the accident

Fatality #4 - March 24, 2010Falling Material - New York - SaltCargill Deicing Technology a BU of Cargill Inc - Cayuga Mine • On March 24, 2010, a 63 year-old contract truck driver with 21 years of experience was fatally injured at a surface area of an underground salt mine. The victim was loading his truck under a 150 ton salt bin when it collapsed, falling onto the cab of the truck. A second victim working in the area received serious injuries.
Fatality #5 - May 5, 2010 Machinery - Alabama - Limestone, Crushed & BrokenChemical Lime Company Of Alabama LLC - Brierfield Quarry • On May 5, 2010, a 21 year-old old contract driller with 1 year of experience was fatally injured at a crushed stone operation. The victim was repositioning a truck mounted drill. He lowered the mast and raised the leveling jacks to move the drill. The drill then rolled down a grade and struck him.
Fatality #6 - May 14, 2010Exploding Vessel Under Pressure - Tennessee - CementCemex Construction Materials Atlantic, LLC - Knoxville Cement Plant Cemex • On May 14, 2010, a 35 year-old mechanic/welder with 4 years of experience was fatally injured at a cement operation. The victim was using an oxy- acetylene torch to cut a damaged drill steel to salvage the drill bit. The drill steel exploded causing metal fragments to strike the victim.

The victim entered a vertical roller mill without locking out the electrical power switch. The mill was started with the victim in the mill.
Fatality #8 - May 26, 2010Fall of Face, Rib, Side or Highwall - Utah - Uranium Ore Denison Mines (USA) Corp. - Pandora Complex • On May 26, 2010, a 28 year-old contract miner with 10 weeks of mining experience was fatally injured at an underground uranium mine. The victim was scaling a rib when he was struck by falling material approximately 11 feet high, 15 feet wide and 4 inches to 30 inches thick.
Fatality #9 - May 28, 2010Explosives and Breaking Agents - California - Gold Ore Gold Run Enterprises LLC - Discovery Day Mine • On May 28, 2010, a 59-year-old supervisor with 20 years of experience was fatally injured at an underground gold mine. The victim and another miner entered a blast area when a misfire detonated without warning. The other miner was injured and hospitalized.
Fatality #10 - June 12, 2010Slip or Fall of Person - Minnesota - Sandstone, Crushed and Broken Unimin Minnesota Corporation - Ottawa Pit and Plant • On June 12, 2010, a 46-year-old contractor welder was fatally injured at a crushed stone operation. He was preparing to weld on an overhead ventilation duct. The victim was using a ladder to access the duct when he fell over a handrail approximately 45 feet to the ground.
Fatality #11 - June 18, 2010Fall of Roof or Back - Idaho - Silver OreU S Silver Corporation - Galena • On June 18, 2010, a 29 year-old contract miner with 6 years of experience was fatally injured at an underground silver mine. The victim was scaling loose ground in a stope when he was struck by falling material approximately 3½ feet long by 2½ feet wide by 2 feet thick.
Fatality #12 - June 20, 2010Powered Haulage - Arizona - Copper Ore NECAsarco LLC - Ray • On June 20, 2010, a 52 year-old mechanic with 8 years of experience was fatally injured at a surface copper operation. A ½ ton pickup truck had parked in front of a 240 ton haul truck that was also parked. The haul truck pulled forward and struck the pickup truck fatally injuring the driver and seriously injuring another miner.
Fatality #13 and #14 - August 12, 2010Falling Material - Nevada - Gold OreBarrick Goldstrike Mines Inc. - Meikle Mine • On August 12, 2010, a 38 year-old maintenance technician with 3 years of experience and a 47 year-old operations miner with 21 years of experience died at an underground gold mine. They were working from the top of a conveyance in a 16-foot diameter ventilation shaft attempting to locate and free a blockage in a 24-inch-diameter aggregate delivery pipe. While the conveyance was near the 820 foot level, the entire pipe from the shaft collar to the 860 level broke away and fell to the bottom at the 1330 foot level. The pipe struck the conveyance as it fell, causing the hoist drum to break away from its support base. The victims were found at the bottom of the shaft.

The victim and another miner were pulling a small boat from a dredge pond onto a boat trailer attached to a pickup truck. When the boat slipped back into the water, the victim attempted to retrieve it and drowned.
Fatality #16 - October 10, 2010Electrical - Oklahoma - Dimension StoneMartin Marietta Materials, Inc. - Snyder • On October 10, 2010, a 42 year- old contract electrician with 4 years of experience was seriously injured at a granite operation. The victim and two co-workers were installing ground fault indicator lights in a circuit breaker enclosure when an arc flash occurred. The circuit breaker enclosure contained a bottom feed circuit breaker. All three workers were hospitalized and the victim died on October 12, 2010.

The victim dismounted the dozer he was operating and walked near a haul truck that struck him.
Fatality #18 - October16, 2010Machinery - Kansas - Limestone, Crushed and BrokenN R Hamm Quarry Inc - Plant #80013 • On October 16, 2010, a 52 year- old haul truck driver with 5 years of experience died at a crushed stone operation. He was using an air-powered hammer/chisel to clean hardened material on a belt conveyor tail pulley. The victim was positioned on top of the return side of the belt conveyor, facing the tail pulley, when the belt conveyor was energized, entangling him in the tail pulley.
Fatality #19 - October 20, 2010Powered Haulage - Georgia - Fullers EarthOil-Dri Corp Of Georgia - Simpson • On October 20, 2010, a 63 year- old contract truck driver with 41 years of experience died at a fuller's earth (clay) operation. The victim backed his trailer into a bay at the mine loading dock. He got out of his truck and walked to an adjacent bay to discuss the loading procedures with the fork lift operator. At that time, a second trailer was being moved into the bay and it struck the victim, pinning him against the loading dock.
Fatality #20 - November 13, 2010Powered Haulage - Texas - Sand & GravelHutchins Sand & Gravel - Hutchins Sand & Gravel • On November 13, 2010, a 42 year- old contract mechanic with 23 years of experience died at a sand and gravel operation. The victim was underneath a front-end loader, with the engine running, checking a hydraulic fluid leak when the machine moved and rolled over him. The machine was parked on a slight grade, the bucket was raised off the ground, and no wheel chocks were in place.
