A different approach Lee Davies – Aster Living
The Conundrum • Unplanned hospital admissions is the biggest shared problem for Health & Social Care. • Quantifying the impact of preventive measures remains subjective. • There is a movement challenging what is classified as a preventive services.
The Conundrum continued…… • Challenging our ability to intervene at the most appropriate point. • Challenging our methods and ability to change our offering to make a greater impact!
The rationale to how we designed our approach • How can we intervene earlier? • How do we justify the DFG process for works which are between £1000- £4000 • How can we support our commissioners to find efficiencies?
Our approach • A new pathway for works between £1000-£4000. • Dedicated response teams. • Referral to completion offering of 3 weeks • Measurable saving for L.A by reducing the need for back office functions.
Added value benefits • The more homes we adapt for less, the more virtual extra care communities we organically create. • The closer we are to this….
Change • Change is among us and changing the culture of what we offer is critical. • Housing options • Minor adaptations • Assistive technology • Redesigning DFG processes
