CHANGE REPORTING OVERVIEW The purpose of the change reporting function in the Access Health CT website is to provide users with the ability to update pertinent information. Keep this in mind: Each time a consumer reports a change to an application a new “Application ID” is created. If you make a change follow it through to the point at which eligibility is determined. Use MAGI as a guide when entering income Information. To enroll in another plan contact the Call Center Note: Use Anonymous Browsing for shopping options; not Change Reporting!
CHANGE REPORTING: TWO TYPES of CHANGES • There are 11 options to choose from on the screen in order to make changes. • In our review today we will cover two common changes. • Add a household member. • Add income from another household member. • (Based on feedback we can cover other scenarios in the future.)
Topic Report a Change: Build Household - Change #1: Add Member The screen to the left is used to report a change. Changes include life events of the primary applicant or his/her household. If a person is making a change for the first time on the application, the link “Report A Change” in income or Household will be displayed. Report a change in Income or Household
Topic Report a Change: Build Household – Change #1: Add Member “Report a Change Screen” indicates some of the input documents needed to provide the information needed to report the change. For example, birth certificate, marriage certificate, pay stub,1040 tax form, company statement. Click “Next”.
Topic Build Household – Reason for Changes “Reason for Changes Screen” Click on the type of change(s) desired. select all changes that you will be making. In this example, the consumer selected “Add or remove household member” and “Change household income or deductions” Click “Next”.
Topic Report a Change: Build Household – Change #1: Add Member “Household Members Screen” lists all current household members. Review that current member are listed correctly. On the bottom right, click “Add Member”.
Topic Build Household – Change #1: Add Member Add Household Members Screen Add a household member include name, date of birth, social security number, gender, answer question on need for health coverage, answer whether social security number is the same, reason for addition, date of change. Fields with red asterisk are required. Just a reminder - click the box “Don’t have social security number.” if the new family member (such as a newborn) has not yet received a Social Security #.
Topic Build Household – Change #1: Add Member Add Household Members Screen Fill in the data boxes We’ll choose “Birth” as the reason for change. (This is one option in the drop down menu birth, adoption, placement for adoption, marriage, and other.) Make sure to enter date of birth (for example, 01/01/2014). Click “Save”
Topic Build Household – Change #1: Add Member Verify that the new household member has been added correctly. Click “Next”.
Topic Build Household – Change #1: Add Member SSN and Identity Verification Advisor Screen. Must confirm with clients that their Personally Identifiable Information (PII) including social security number will be used to verify their identity. Click “Next”.
Topic Build Household – Change #1: Add Member Household Address and Contact Information Screen Unless the inclusion of a new family member changes this information, just click “Next” .
Topic Build Household – Change #1: Add Member Application Review Screen Review the information on the screento ensure accuracy. Click “Confirm”.
CHANGE REPORTING Have created a new household member. Next step is filling-in individual details. This information is added to ensure correct eligibility, and includes: • State residency • Pregnancy • Disability • Citizenship/Immigration • Tribal status Note:the system would normally display each household member’s information, for clarity those redundant screens will not be shown as part of the review.
Topic Individual Details - State Residency - Change #1: Add Member State Residency Screen This will confirm whether the new family member resides in or outside the state. (Example where this is pertinent: recently divorced parents with a new child). Click “Next”.
Topic Individual Details – Pregnancy - Change #1: Add Member Pregnancy (Example where this is pertinent: a new spouse). Click “Next”.
Topic Individual Details – Disability Status - Change #1: Add Member Disability Status (Example where this is pertinent: An adopted child with disabilities). Click “Next”.
Topic Individual Details – Citizenship/Immigration - Change #1: Add Member Citizenship and Immigration Status Screen” Review screen, either keep as selection appears or change. Going back to the original example, check off Veronica as a US Citizen, Note: Consumer will be instructed to submit verification documentation later in the process. Click “Next”.
Topic Individual Details -Tribal Status Change #1: Add Member From “Tribal Status Screen” Answer as circumstances dictate. Click “Next”.
Topic Individual Details - Change #1: Add Member Individual Details Screen . Answer as circumstances dictate. To information, must select “Add/Edit” Click “Next”.
Topic Individual Details - Household Relationships - Change #1: Add Member Household Relationships Screen(this screen will require input to complete). Verify correct relationships. At the screen to the left you can see our initial example, Veronica Fekete, and her various family relationships. (For example: Veronica Fetke is the daughter/son of Lon). Click “Next”.
Topic Individual Details -Application Review - Change #1: Add Member Application Review Screen Review screen – check for accuracy. Click “Next”.
CHANGE REPORTING - Process check - Stated that this session focused on an example that made two changes– “add a family member” and “change household income”. If the second change was not selected, the system would go to Eligibility Status for the new family member. Because of the second change, the system flow will continue as follows.
Topic Individual Details - Tax Information – Change #2: Income Change Federal Tax Details Screen Select the correct filing status and identify which tax payer is the primary filer Click “Next”.
Topic Tax Information – Change #2: Income Change Federal Tax Details Screen(#2) Complete for the secondary filer. Click “Next
Topic Tax Information – Change #2: Income Change Federal Tax Details Screen Answer the question “Will every member in your household have the same tax status in 2014 as in 2013?” If “Yes”, click Yes Click “Next’.
Topic Tax Information – Change #2: Income Change Federal Tax Details Screen Click “Filing Status” for (the new family member) Veronica Feteke. Assuming Veronica is a newborn, select that she is a “Tax Dependent of Someone from Household” and choose which family member she is a dependent of (Lon Feteke). Click “Next”.
Topic Tax Information - Application Review – Change #2: Income Change The “Application Review Screen – Tax Filing Status” is displayed. Click “Confirm”.
Topic Tax Information - Income Confirmation – Change #2: Income Change Income Confirmation Screen In this example, the change of income does not come from the ‘primary job’, but from ‘other income. Click “Next.”.
Topic Household Income– Household Income Types – Change #2: Income Change Household Income Types Screen Answer the question “Are you or someone in your household currently receiving other income?(changes would be made here) Click “Yes” Click “Next”
Topic Household Income – Current Month Income - Change #2: Income Change Household Income Types Click “Add Income”. Add new income source (Sample of pop-up that is created when clicking “Add Income” not available). When finished adding “other income”, click Next. Other income $XXX.YY/Month
Topic Household Income - Household Income Types - Change #2: Income Change HouseholdIncome Types Screen A reminder – this is where a change to self employment would be made.
Topic Household Income - Change #2: Income Change Current Deductions Screen Answer deductions question. If, “Yes”, identify who is getting the deductions. Click “Next”.

Topic Additional Questions- Employer Offered Coverage – Changes #1 & #2 Additional Questions – Employer Offered Health Coverage Screen Had members of the family had changes in insurance through an employer health plan, this screen would allow for changes. Click “Next”.
Topic Additional Questions - Application Review – Changes #1 & #2 From “Application Review -Additional Questions Screen” Had there been any, this screen displays any information that may have been entered on employer offered insurance. Click “Confirm”.

Statement #5 acknowledges that by inputting changes, eligibility may change. Click “Next”.
Topic Application Review - Enrollment Qualification – Changes #1 & #2 From “Enrollment Qualification Information Screen” (This screen displays eligibility information.) Click “Next”.
Topic Eligibility Determination - – Changes #1 & #2 Eligibility Determination Screen This screen displays eligibility information for all household members. Consumer clicks insurance selection of their choicefor their new family member. Click“Next.
CHANGE REPORTING SUMMARY The Report a Change Function enables changes to be made to an account. These changes range from : • Adding a Household Member, • Changing Address • Changing Citizenship/ Immigration Status • Income Changes • Tax Information • The above changes may impact eligibility
