Presents request to AD/GE subcommittee Denied Return form to faculty member Approved request forwarded to Curriculum Committee Approved request forwarded to Board of Trustees Paperwork returned to Academic Affairs The Office of Academic Services will work with the faculty member/department to complete the required forms for State approval. For new programs and certificates, the CCC-501 form is used and for changes to an approved program or certificate, the CCC-510 form is used. Faculty member meets with Articulation Officer (AO) or AD/GE Rep. to discuss appropriate certificate app. Faculty member meets with Articulation Officer (AO) or AD/GE Rep. to discuss next step if a denial is made. Approved request forwarded to Off. of Academic Services Approved request forwarded to CCCO for State Approval Approved completed request form forwarded to AO for filing • Notes: • * Only Certificates that are approved by the CCCO (Chancellor’s Office) will be posted on the students’ transcripts. ONLY Type II and Certificates of Completion may be submitted to CCCO. • 2. Credit and Non-Credit Certificates of Completion submitted after March 1st will not be published in the next year’s catalogue due to publishing deadlines. • If a low unit Certificate of Accomplishment (12-17.99 units) is to appear on a transcript, once local approval has been obtained, additional paperwork must be completed prior to State approval. Contact the Office of Academic Services for forms and additional information. • Certificates over 18 units are Certificates of Achievement.
