  2. Disclosure • Please state your conflict of interest on Slide 2. • If you do not have a conflict of interest make that statement as well on the slide.

  3. Objectives • Please list your objectives as listed on your abstract. • Do not change your objectives – if so, it requires a review by the program planning committee and your submission may be denied. • Objectives are listed on Slide 3.

  4. Begin your presentation here. • Existing slides above are required. Do not remove or re-order the first three slides. • Use the Master Slide view for format/slide options. • Click ‘New Slide’ and select slide style to be inserted. • Copy and paste (CTRL C, CTRL V) existing slides to add additional like slides, i.e. polling questions; same headings. • Use existing fonts, style, and color schemes provided. • To blank the ‘background’ of a slide, i.e. for large tables or graphics, right click, select Format Background, and click ‘hide background graphics.’ • This slide may be deleted.
