Form 1 • If applicable, we recommend to submit this form first because • It’s a bargain: With $120 + tax (i.e. $135 in CA) and this form, you will receive the new club charter kit, which includes 20 copies of CC and CL manuals ($280 + tax value), and basic club supplies • Help you start the club faster: It’s easier to convince interested parties to sign-up with the manuals available. The earlier we get 20+ people signed-up, the earlier the club can kick-off Thrive in District 5 http://d5tm.org

You may call Toastmasters International at 949-858-8255, asking for “new clubs” to provide the financial information. • 2 –The only special category is “Advanced Toastmasters”, but this can be easily changed later, and there’s no significant difference among the categories at this point • 3 – Pick one language. The soft copy is available for free download – www.toastmasters.org/clh • 4 – Pick one language • 5 – You may leave this portion blank. 20 kits will be sent if only this form is submitted. If all forms are submitted together, the kits number can be indicated on form 2 • 6 – You may leave this portion blank. Form 2 (Page 1 of 1) • 1 – The name can be changed online later, but this will be the name to be displayed on your first club banner (unless you change it before the banner is ordered) • 2 – Should match form 1 • 3 – You may leave this portion blank. Your district director can fill-in this information later. • 4 – • New member: the person has never been a Toastmasters member before, it’s required to get a new member kit for new members • Dual member: the member is currently a member of another TM club and plan to join this club as well. New member kit can be purchases if needed (optional) • Reinstated member: the member was a Toastmasters member before. Form 6 • Contact your club sponsors (mostly assigned by the district ) or district officers if you have concerns about the items in p1-6, or if you need further clarification on the questions in p7-8 • Your answers on p7-8 can be easily changed online after the club is chartered.
