2. Background and Rationale The NHS Improvement Plan published in June 2004 introduced the Government’s ambition that “by 2008 no-one will wait longer than 18 weeks from GP referral to hospital treatment.”
The 18 week pathway focuses on the delivery of an efficient, effective and safe patient journey from initial GP consultation and referral to the start of treatment, and includes waits for GP referrals, consultant led services including consultant-to-consultant referrals, A&E and Walk-in Centre referrals as well as any therapies that form part of the patient’s treatment.
Key targets and milestones have been identified by the Department of Health to enable organisations to scale and manage the changes in working practice, management and measurement required.
Delivery of the 18 week milestones is one of the core targets within the South Central SHA Annual Plan. The SHA has the opportunity to proactively support PCTs and Trusts in the delivery of these changes through a range of interventions.

This programme aims to:
Deliver the December 2008 target by March 2008
Deliver improved patient experience and safety at lower cost through effective resource usage and improved patient information and communication.
The Programme Objectives are to:
Build the capability within organisations to manage demand and capacity efficiently and effectively, to ensure the best patient care is delivered in the most appropriate setting and location at an affordable price
Define and implement new pathways for delivering efficient, safe, high quality patient care, removing waste and ensuring all activities are high quality and patient-focused
Create Service Improvement capability in South Central through the transfer and development of skills in pathway redesign, process improvement and quality management, project and programme management;
Build leadership skills in those holding executive positions in organisations across the SHA
