2. Current social status and forecast in Japan • Japan is very rapidly becoming so called an aging society • Increase of average life span and decrease of birthrate • Declining birthrate ⇒ Decreasing worker population ⇒ Revenue shortages 2. Aging ⇒ Increase in social security costs 3. Keep a balance between satisfaction and financial resource.

3. IT structural reform strategy • Issued in January, 2006 by Government of Japan • Structural reform enforced by ICT as leverage • Solution to the foreseeing social dilemma • Focus onHealthcare and e-Government • Healthcare • Construction of National database for EBH, EBM and personal EHR • Supporting home medical treatment and consultant using ICT etc. • e-Government • Establishment of GPMO, PMOs to optimize the total e-Government • Establishment of IT governance etc.

4. Promotion of IT in Healthcare • Introduction of personal healthcare record system (EHR) to enable retrospective examinations (personal benefits) and to establish a preventive medicine (public benefits) • Home healthcare using digital TVs • Medical cost settlements will be 100% paperless by 2010  Estimated saving of administration cost is 1400M US$/year  Next IT investment

5. New pension management system plan(Announcement from The government party) • Introduction of Social Security Card • A card having roles of a pension booklet, health insurance card, and nursing care insurance card etc. • Considering the health insurance card for example, the number of card is 110 million for people of age 10 or older. • Facial photo is attached if required as photo identification card • Allows you to check your own pension record safely and promptly anytime not only from the contact window at the office, but also from PC in your home. • Linkage with the resident registration network • Security is ensured so that no one can steel another persons' record. • It is planned that Social Security Cards start to be delivered in 2011.

6. Introduction of e-P.O. (Post Office) box (1) • Purpose • Disclosure of information on a person to the person by administrative and social security-related organization • Background • Proposed in the IT Strategy Headquarters of Dec. 2006. • Described in the Priority plan 2007, Large-boned policy of the government. • The support body will be established newly in the IT Strategy Headquarters; Cooperation by the Cabinet Secretariat, Ministry of Internal Affairs and Communications, and Ministry of Health, Labor and Welfare. • Basic policy will be decided in 2007, environmental arrangement will be made in 2008, and the e-P.O. Box will be implemented in 2010.

7. Introduction of e-P.O. box (2) • Open a personal account in the cyberspace for every Japanese resident • Just like a bank account, Smart card will be used for a secure and easy access  Good for the social acceptance • Convergence  all services provided through the portal • e-Government services; One stop service • Social security status check; national pension, health insurance, employment insurance etc. • Healthcare; EHR for better preventive medicine

Conclusion • It is necessary to study, ASAP, introducing the e-P.O. box which has combination with new e-ID card. - All messages through this box are standardized, digitally signed and encrypted - Access to this box could be made by eID just like bank account - Life event such as employment, retirement, graduation, move, drives a navigation of administrative procedures -Other data such as EHR could be downloaded at the user’s choice • It is recommended to have the resident registration card and social security card coexist and to support access to the e-P.O. box by those cards as new e-ID Card through the use of public personal authentication service. • Such a system should be studied in which personal information, upon a request by that person in the standard description format, is provided to the e-P.O. box of that person, and the sender of the information bears the cost.
