SNHSA/MW Medal Finals October 28 - 29, 2006 Entries close Friday, Oct.. 27, 2006 Please mail entries to: Laurie Alden 30495 Lakeshore Dr Washoe Valley, NV 89704 Phone: (775) 846-9501 CLASSES ENTERED Classes $15 x Apple Bobbing $5 Costume Class $15 Grounds Fee $5 SNHSA Year End Awards $5 Late fees $5 x TOTAL Riders must have qualified at previous shows to ride in all medals (classes 13, 14, 24 and 26). Parents are encouraged to participate in the apple bobbing! Don’t forget the costume class! No phone or faxed entries accepted. Entry form must be filled out completely and received along with entry fees on or before Friday, Oct. 27, or post entry fees will apply. Post entry fees are $5/class. Each horse will be charged $5 for SNHSA Year End Awards and a $5 grounds fee. I, the undersigned, a participant in the equestrian activities at Maplewood Stables, Inc. agree to and hereby assume the risk for any injuries that I may sustain in the pursuit of such activities while on these premises and hold harmless Maplewood Stables, Inc., Mike and Julie Winkel, their employees, agents or representatives, paid or unpaid, from any liability or any personal injuries, death, damage, theft or any other loss which might occur to my person, family, friends, horses, personal belongings, or animals. MUST BE SIGNED! (By parent if under 18 yrs.)
