The Next Six Months • Continue and build practice engagement. Encourage nonparticipating physicians and NPs to join your work. • Registry: If you have one, figure out how to incorporate its use into practice flow. If you don’t, continue to work within your organization or practice to get this capability. Use billing inquiries to at least know your denominator and who you are missing. • Sustain achieved improvement. Update flow diagram as needed. Analyze failures. Monitor processes.
