Target Area? • Start-target older adults in St. Paul Wards 1, 3, 4 • Then-Union Park, Macalester Groveland, Summit Hill, Summit University, Highland Park, West 7th • Then 55012, 55103, 55104, 55105, 55116 • Now • Local area • Ramsey County and surrounding area
Who to serve? Start • Aiming to serve both healthy and frail older adults • Private pay and health plan reimbursement • All in target area Now • 87 clients (47 UCare/40 independent) as of 6/1/2011 • Medica contract (non-dual plan) start 7/1/2011 • Lots in enrichment & wellness (187 people) • 367 volunteer hours, 1,236 volunteer miles driven
Collaborators? Start • Lot’s of organizations large and small • Try to work with all at once • Informal agreements vs. formal relationships • Local advisory group and planning group met Now • Collaborate 1:1 between organizations • Contracts and MOUs dictate terms of relationships • Money actually changing hands • JFS, MAAA, UCare, Medica, independent LAH/BNPs
Sustainability Start • Grant monies (obviously) • “We’ll get health plans to pay” • Concepts for private pay • Decided to hire Ingenix consultant Now • Case management stream • Health promotion stream • Dual and non-dual health plans • Primary care (Med B, Minute Clinic, Plans) in works • Subscription fee “concept” under discussion
What to do? Start • Coordinate health care/services • Use communication systems and technology • Provide housing and services to keep out of SNF • Use Vital Involvement Now • Coordinate health care/services • Use different technology, mostly web-based • Coordinate vs. provide housing and services • Lot’s of V.I., Patient Activation, Increase Engagement
When do we actually start? Start-to date • Won RFP summer 2009, contract Jan 2010 • Started “practicing” in April 2010 • Formal “pilot” started November 2010 • First paying contract April 2011, second July 2011 June 2011 and forward • Health Care Home by end of 2011 • Collect meaningful data for evaluations • Build model for simulation to optimize resources • Fully cover expenses by April 2012 (about 30% now)
How to measure success? Start • DHS sponsored evaluation • Looking at Level 2s and 3s • Use 90 day evaluation on all Also • Wilder Evaluation • Model & Simulation • Process evaluation • Health Plan and Health Care Home Quality
