Primary Care • Where we were • No LD Registers • DES for LD • Registers compiled by personal knowledge, computer search
The Past • Terms included • Mental retardation • Educationally subnormal • Learning Difficulties • Trisomy 21
BMJ2008;337:a2507 • The key point is that people with LD have twice as many health problems than the general public, yet their health needs are often unrecognised and unmet.
Examples of health problems commonly missed in LD • Hypothyroidism and coeliac disease in Down’s syndrome • Sensory impairment • Mental health problems • Dementia
Constipation • Gastro-oesophageal reflux • Poor oral health
Why do people with LD have poorer health? • Greater risk of exposure to the social determinants of poor health • Increased risk with specific genetic and biological causes of LD • Chronic health problems are more common
Communication difficulties • Deficiencies in access to, and quality of healthcare
Annual Health Checks • Aim to detect unmet, unrecognised and potentially treatable health conditions • Aim to target actions to address these health needs
For example, Cardiff Health Check Study,2006 • 51% of patients with LD had health needs identified • Of these,37% had 2 or more health needs • 9% of patients had a serious health problem
