What is an Advance Directive? An Advance Directive is a legal document that allows anyone over 18 years of age to: • Appoint a health care representative (agent) and/or • Give a set of instructions for care (in the event the person should lose their decision-making capacity).
An individual does not have to: • Appoint an agent (Part B) • Give instructions (Part C) • One or the other is permitted. • If they do both, the authority of the agent is usually understood to be limited by any instructions.
More than an Advance Directive This is an order for patient’s for whom, because of underlying disease or age, it would not be a surprise if some life-threatening event (cardiac/respiratory arrest, pneumonia, loss of swallow) came about from which the patient would not want to be, or could not be, rescued.
