  3. Problem / Issue New Brunswickers are disproportionately affected by chronic diseases 77% Chronic diseases 63% obesity 35% Diabetes & prediabetes

  4. Measurement 3500 Group Coaching Participants Provincially 73% of clients have 1 or more self reported chronic disease 6 Coaches 6 Regions Over 1000 Coaching Clients 53 Average Age 30% female 20% male

Intervention / Actions • Self-Management Support Coaches are connectors They link their clients to resources in their community Coaches teach their client how to set and maintain wellness goals Coaches facilitate positive behaviour change. • Continuity • Bridge Between Clinician & Patient • Five Roles of a Health Coach • Emotional Support • Navigation of the Health Care System
