  4. Implementation Identified processes that needed to be altered prior to implementation New Clinical Respiratory Scoring Tool Discharge checklist Updating Home Management, Plan of Care, etc.

  5. Metrics / Data Management Working with Quality to incorporate CHAT outcome measures Identifying key players to help in data collection
