Our principle partners are the Borough Council, the Health & Well Being Board, Darlington Partnership, Darlington Healthwatch, neighbouring CCGs, and councils, NHS England, practices, patients and the public. • To secure the right services in the right place for the people of Darlington • [Emergency admissions for children with lower respiratory tract infections] • [Unplanned hospitalisation for asthma, diabetes and epilepsy in under 19s] Working together to improve the health and well-being of the people of Darlington Ensuring that people have positive experience of care Treating and caring for people in a safe Environment and protecting them from harm Preventing people from dying prematurely Enhancing quality of life for people with long-term conditions. Helping people recover from episodes of ill health or following injury Securing Quality in Hospital Services (SeQIHS) / Primary Care Strategy (Whole Systems Approach) / Quality Handover Quality, Innovation, Productivity, Prevention programme Developing the Clinical Commissioning Group and its members Patient safety and clinical quality/Implementation of Francis 2 / Winterbourne recommendations • Reconfiguration of urgent care\A&E services     • Develop and implement revised arrangements for the care of children   • Delivery of the ‘dementia ambition’ and implement programme of service reviews    • Email rapid advice service     • Increase in • non-elective • activity above • affordable level • To manage our resources effectively • Implement Effective demand management plans through practices     • On going critical review of contracts     Version 9.1 23/07/2013
