e-Prescribing • Latest figures show 85% of US Pharmacies are capable of using e-prescribing • 25% of prescriptions are written electronically • e-Prescribing requires a clearing house agency: Sure scripts • e-Prescribing is part of Meaningful Use Services
Benefits of e-Prescribing • Improved Documentation • Expedites filling of prescriptions, reduces admin time in handling refills by nurses or refill lines • Improves Medication reconciliation, including from most outside pharmacies • A requirement of CMS e-prescribing incentive program that we must meet to secure UNMs participation
Benefits of e-Prescribing • Verifies Formulary Drugs through same data base • Allows review for need of refills at the time of visit, thus reducing call backs • Prescription is generally ready at time patient presents • Reduces Medical Errors, drug interactions
Reduction of Medical Errors by Electronic Prescribing • Estimated 100,000 individuals die of medical errors each year. • Systematic Review of 25 studies that addressed Medical Errors • Study design was variable, almost all showed reduction in errors decreased by 30%-83% • Meaning of these findings for patient outcome is not determined Ammenwerth, et al JAMIA 2008
Phoned Prescriptions at UNM • Study to track phoned in prescription not recorded in EMR • 2 three week periods studied • Total of 217 prescriptions were called in with 53% not recorded in PCO • Internal Medicine 38%, FM 18% and Psych 20% • e-Prescribing did not impact how many prescriptions are called in during study period
UNMH ePrescribe • Implemented at UNMH June 13, 2011 • Current Statistics • 3,000 prescriptions written daily • 1,000 are ePrescribedi.e. electronically sent to a pharmacy • 3-5 errors experienced per day • pharmacy staff monitors these errors and are responsible for follow-up
Karen Day, Instructional Analyst • Description of how e-Prescribing works • Short Training Course for e-Prescribing feature through Power Chart • Questions
Recommendations • Commit to use e-prescribing preferentially over hard copy prescriptions (except for scheduled drugs for now) for discharging patients • Send prescriptions the day before discharge to ensure prescriptions are ready • Must stay current (prescribe within 180 day period) or your name may be deleted from clearing house data base
