  4. Twee RCT’s Vaginale prolapsoperatie versus prolapsoperatie met TVT/TOT • Bij predominant SUI (CUPIDO-1) • Bij occulte SUI (CUPIDO-2)

  5. Wordt met een gecombineerde ingreep de kans op postoperatieve SUI minder zonder toename neveneffecten (complicaties, urge enz) ? CUPIDO

  6. POP & SUI • 40% predominant SUI • CUPIDO-1 • 60% niet/nauwelijks incontinent • Daarvan 30-80% occulte SUI • CUPIDO-2

  8. ≥ Stadium 2 POPQ prolaps en SUI Predominant SUI: symptoom (>1x per wk) of teken (+ hoesttest) van SUI SUI > UUI Geen geïsoleerde achterwandprolaps Sample size: 65 vrouwen per arm (130) CUPIDO-1

  9. ≥Stadium 2 prolaps Waarvoor vaginale prolapsoperatie • Wilsonbekwaam, < 18 jaar • Zwanger of nog zwangerschapswens, < 1 jaar pp • Prolapschirurgie < 6 mnd geleden • Eerdere OK blaas/urethra/TVT/ Burch • Bekend blaas-/urethradivertikel • Systeemziekte die blaasfunctie kan beïnvloeden • Geplande chemo-/radiotherapie • Deelname andere studie die resultaten kan beïnvloeden • Overloopblaas of chronische retentie Exclusie criteria >1 keer per week stress incontinentie OF positieve hoesttest zonder redressie Nee Nee Nee Ja Ja Predominantie SUI?   11. ≥ Stadium 2 POPQ prolaps ≤ 1x per week klachten van urine incontinentie Negatieve hoesttest SUI na gestandaardiseerde redressietest op poli en/of bij UDO CUPIDO-2

  12. Sample size: 80 vrouwen per arm (160) UDO obligaat (diagnostische studie) 50% occulte SUI en daarmee 320 UDO’s Indien geen occulte SUI alleen prolaps operatie  follow up Indien wel occulte SUI: randomisatie CUPIDO-2

  13. ≥Stadium 2 prolaps Waarvoor vaginale prolapsoperatie • Wilsonbekwaam, < 18 jaar • Zwanger of nog zwangerschapswens, < 1 jaar pp • Prolapschirurgie < 6 mnd geleden • Eerdere OK blaas/urethra/TVT/ Burch • Bekend blaas-/urethradivertikel • Systeemziekte die blaasfunctie kan beïnvloeden • Geplande chemo-/radiotherapie • Deelname andere studie die resultaten kan beïnvloeden • Overloopblaas of chronische retentie Exclusie criteria Hoesttest met redressie + UDO verrichten ≤ 1x per week urine incontinentie EN Negatieve hoesttest zonder redressie Nee Nee Nee Ja Ja Occulte SUI?
