About us – Garden River Wellness Centre We are an Ojibway First Nation Community providing health & social services to 1200 on-reserve members; We have 28 full-time and part-time staff plus 13 visiting regulated health professionals who deliver primary health care and other health & social programs and services;
Our Program Delivery Model Our programs/services are broken down into four main areas based on the medicine wheel concept: Community Outreach Services Admin./Financial Services Physical, Child, East, Spring … Spiritual, Elder, North, Winter … Health Services Social Services Emotional, Youth, South, Summer … Mental, Adult, West, Fall …
Our Programs/Services • GRWC delivers Health & Social Services from Provincial & Federal Government Funding Sources; • Administrative/Financial Services: human resources; clerical services; accounts payables/receivables; and payroll. • Community Outreach Services: activities to promote wellness; children’s oral health services; cultural awareness and services; and prevention programs i.e. sickness and disease; • Social Services: addictions and mental health services; child and family services; speech and language services;crisis intervention & referral and court representation for child welfare matters. Our Programs/Services (cont’d) Health Services: chiropody/foot care; chronic disease management;; emergency preparedness; environmental health; homecare for elderly & disabled; non-insured health benefits; dietitian; occupational therapy;physiotherapy; prenatal and postnatal care; primary health care;traditional health services; and transportation.
Our programs & services were designed holistically but our record keeping systems were not! Electronic Record - Goal • To implement a Holistic Patient Registration, Scheduling and Electronic Record for all clinical and non-clinical programs/services provided at the Garden River Wellness Centre • To build a model based on our holistic approach hence naming of the Electronic Record – Medical, Health did not define our needs – therefore the “Electronic Wellness Record”
Our Unique Electronic Record • Every provider counted: • Privacy and Security issues were addressed during mapping of roles and responsibilities of both clinical and non-clinical staff including Traditional Healer, Prevention/Promotion workers; Centralized patient/client registration for every provider; • One Client - One Record • Improved communication between providers; • Unique Status Number displayed in demographic screen; • Simplified a reporting “nightmare” i.e. Federal, Provincial funder requirements;
