  4. 1) Supporting 21st century healthcare The LIFT Programme drives significant improvementsin the quality of health service accommodation Improving working conditions and facilities for a wide range of service providers Replacing inadequate premises with high quality,flexible accommodation

  5. 1) Supporting 21st century healthcare Increasing service integration and improvingjoint working between heath and social services Helping to reduce unnecessary hospital appointmentsand facilitate more efficient referrals Enabling a more holistic approach to the deliveryof health and community services

  8. 2) Improving the health and wellbeing of our communities The LIFT programme deliversmodern and flexible facilities, designed to meet the diverse health and wellbeing needs of disadvantaged and often rapidly changing populations In areas where a significant numberof residents suffer from socio-economicdisadvantage including poor health and wellbeing.

  9. 2) Improving the health and wellbeing of our communities Provision of new community settings for services has also led to greater take up. LIFT has brought care closer to patients and is facilitatingmuch better access to a greater range of services available under one roof, resulting in improved quality services LIFT has also enabled other programmes aimed at promoting healthier lifestyles and more outpatient activity.

  10. 2) Improving the health and wellbeing of our communities Even a relatively small centre such as Sedbergh in Cumbria, contains consulting rooms, ‘soft rooms’ for counselling services, treatment roomsfor minor surgery, facilities for physiotherapy and chiropody, a small ward for Sedbergh school and training/conference space.
