The Integration Transformation Fund • A pooled Budget from 2015/16 with national conditions • Jointly agreed plans • 7 day a week health and social care working around admissions and discharges • Better Data sharing inc use of NHS number • Joint assessments and care planning • Accountable professional for joint packages of acre • Risk sharing and performance targets
The Challenges • Lack of Clarity- Guidance …December 13 • Timescales – First Submission – Feb 14 • Source of Funding – extra £10m each from CCG • Very Hospital Focused – LLR • Performance Related Funding Element • Additional Work within Leicestershire on Transformation and a place based approach ( locality working)
Where are we now in Leics ? • Health and Wellbeing Board • Integrated Commissioning Board • Some Aligned and Integrated Services • Planning Group for Integration Transformation Fund Plan • Work underway on Shared Dataset, Crisis Response, Hospital Discharge, Reablement
Current Ideas • Shared Minimum Data set • Single ( single!!) point of access • Integrating some teams eg Reablement • Joint Approach to Prevention • DFG, Equipment and Assessments
